I write to place all recipient organisations on formal notice that Mrs Irene Yok Moy Lem is a vulnerable person who has already suffered significant harm as a result of previous unlawful or improper enforcement actions taken against her.
Each organisation addressed in this letter is now expressly informed that:
Irene is a vulnerable individual experiencing ongoing distress.
Prior enforcement actions have caused serious psychological and physical harm.
Any continuation of pressure, enforcement activity, or administrative conduct that may intimidate, distress, or disadvantage her would be wholly inappropriate and inconsistent with your statutory obligations, including fairness, proportionality, and the protection of vulnerable persons.
This combined notice ensures that no authority can claim lack of awareness of Irene’s vulnerability or the harm already caused.
To ensure full transparency and accountability, I provide the following materials:

2016–2018 – Builder’s Contractual Responsibility
Irene entered into a HIA Home Building Contract (23 June 2016) with licensed builder Ian Jack, delegating all planning and building compliance responsibilities to him.

8 January 2018 – SAT Mediation Agreement
SAT Mediation Agreement CC2487/2017 executed.
17 May 2019 – Magistrates Court Judgment
Magistrates Court (PER/CTC/4423/2018) awarded $75,182.81 to Irene against builder Ian Jack.
“The Court’s judgment confirms that the builder, not Mrs Lem, was at fault…”
2019–2023 – City of Stirling Enforcement Despite Builder Liability
City of Stirling pursued prosecution and enforcement despite builder liability and Irene’s good faith reliance on the builder.
